The Gut-Hormone Connection: How Your Microbiome Influences Perimenopause


When most women think about perimenopause, they think about hormones.


Hot flashes. Mood swings. Irregular periods. Brain fog. Weight changes.


What often gets overlooked is one of the biggest players influencing how those hormones behave:


Your gut.


As a naturopathic doctor with a special interest in women's health and gut health, this is one of my favourite topics because it highlights just how interconnected our bodies really are.


Let's take a look at why your microbiome may be playing a bigger role in your hormone symptoms than you realize.


Your Gut Helps Regulate Estrogen


Estrogen doesn't simply get produced and disappear.


Once your body has used estrogen, it travels to the liver where it's processed (a process called oxidation and conjugation) and packaged to be eliminated through the intestines. Healthy bile flow is an important part of this process.


Once estrogen reaches your gut, one of two things happens:


  • It leaves the body through your stool.

  • Or it gets reabsorbed back into circulation, increasing estrogen levels.


So what determines which path it takes?


Your gut microbiome.


Meet the Estrobolome


The collection of gut bacteria involved in estrogen metabolism is known as the estrobolome.


Certain bacteria produce an enzyme called beta-glucuronidase, which can "unpackage" estrogen and reactivate it, allowing it to be reabsorbed into the bloodstream.


This process plays a major role in determining whether your body is holding onto too much estrogen, eliminating too much, or maintaining the healthy balance we're aiming for.


I often describe estrogen as the Goldilocks hormone.


We don't want too much.We don't want too little.We want it to be just right.


What Happens When Estrogen Isn't Balanced?


Too Much Estrogen


Higher estrogen levels may contribute to symptoms such as:


  • Heavy or irregular periods

  • Mood swings

  • Weight gain or difficulty losing weight

  • Headaches

  • Acne

  • Bloating

  • Digestive discomfort


Too Little Estrogen


Lower estrogen levels can contribute to:


  • Poor sleep

  • Low mood

  • Vaginal dryness

  • Recurrent urinary tract infections or urinary irritation

  • Low libido

  • Bone loss over time


Why This Is Especially Important During Perimenopause


One of the defining features of perimenopause is that estrogen becomes incredibly unpredictable.



Some months your ovaries may produce two or even three times your usual estrogen levels.


Other months they may produce very little.


These dramatic fluctuations are completely normal—but they can make symptoms feel anything but normal.


While we can't stop this natural transition, we can support the body's ability to adapt to it.


Supporting gut health is one important piece of that puzzle.


Your Gut Bacteria Matter


Not all gut bacteria affect estrogen in the same way.


Some opportunistic bacteria—including certain species of Clostridium, Bacteroides, E. coli, and Staphylococcus—can produce larger amounts of beta-glucuronidase, increasing the amount of estrogen that gets reabsorbed.


On the other hand, beneficial bacteria such as Lactobacillus, Bifidobacterium, and Faecalibacterium tend to support a healthier, more balanced gut environment.


Another fascinating bacterium, Akkermansia, helps maintain the integrity of the intestinal lining, supporting the gut barrier and reducing unwanted substances from crossing into the bloodstream.


The goal isn't to eliminate bacteria—it's to create balance.


Why I Love Microbiome Testing


One of my favourite tools in practice is comprehensive microbiome testing.


Many women assume that if they have regular bowel movements, their gut must be healthy.


While that's certainly a good sign, it doesn't tell us what the microbiome actually looks like.


A stool microbiome analysis can provide valuable insight into:


  • The balance of beneficial and opportunistic bacteria

  • Markers of inflammation

  • Digestive function

  • Gut barrier health

  • Patterns that may be influencing hormone metabolism


Rather than guessing, we can build a treatment plan based on what's actually happening in your gut.


The Bottom Line


Perimenopause doesn't only affect your hormones—and your hormones don't function independently of the rest of your body.


Your gut, liver, microbiome, nutrition, sleep, stress, and lifestyle all influence how you experience this transition.


Supporting your gut won't eliminate the hormonal changes of perimenopause, but it may help your body navigate them more smoothly.
